Instructions
Thank you for your
interest in the Special Needs Registry. This program has been developed
by the Community and Law Enforcement Aware
Response Committee. Formed in 2003 in Santa Clarita, California,
CLEAR is a collaborative of organizations and individuals who are
concerned about the increased risk of special populations in criminal
situations, either as victims or offenders. CLEAR focuses on creating
solutions.
Purpose
The Special Needs
Registry assists law enforcement personnel to be more responsive in
emergencies to residents of the Santa Clarita Valley with special
needs. The registry can help law enforcement identify those who cannot
identify themselves due to a disability or special need, such as
Alzheimer’s Disease or dementia, autism, or a speech disorder. Families
or agencies can voluntarily provide information about their individuals
with special needs so that law enforcement will:

Have a photograph,
description and contact information for people with special needs in our
community who may need special assistance in an emergency.

Be better equipped to
help people with special needs who may become lost, injured, or who may
wander away from home.

Be aware of special
medical, safety and behavioral concerns of individuals with special
needs.

Be aware of
accommodations that may be needed in interacting with the person.
How
to Participate: For residents of the Santa Clarita Valley

Complete a Confidential
Special Needs Registry Form for each individual, providing as much information as you can.

Include a recent
identification-type photo such as a passport photo or school photo.

Check the box to receive
an email reminder when it is time to update the form. Forms not updated will be discarded at the second anniversary so that the Sheriff’s
department will be sure that all the individuals in the Special Needs
Registry are still living in the area and that the information on file
is current. Alternatively, obtain and complete a reminder postcard from
the watch deputy, which will be mailed to you when it is time to update.

The person completing the form should sign and date it. In the case of
adults being registered, the adult should sign the form unless he or she
has a conservatorship.

hand deliver the completed form with photograph attached to:

General Tips

The registry is available
to all residents of the Santa Clarita Valley who are served by the Santa Clarita Valley Sheriffs Station. City residency is not required.

Write legibly, or
complete the form online. You can access the online form at
www.clearscv.org.

When answering the
questions, think about what your special needs individual might do in
case
of emergency and use this information on the
form.

Where will he or she go?
Is he or she drawn to water, busy roads or other unsafe places?
-
Will he or she hide from
emergency personnel?
-
What could a Sheriff do
or say that can help calm them?
-
How will they react to
being touched?
-
What medication will he
or she need?
-
What foods should be
avoided?
-
If the person must be
detained, should he or she be held separately from other jail detainees?
-
What other accommodations
might the person need?
-
If your phone number,
address or other important information changes before your annual
update, we recommend that you complete a new form and mail it in.

Make sure your family
member or client with special needs carries some sort of
identification. Bracelets, necklaces, ID cards, and clothing tags are
all good choices.

Tell other family
members, friends or neighbors about the Registry, especially if they
will be listed as alternate Emergency Contacts.

Keep a copy of your
completed form for your records. The form you submit will be kept in a
confidential binder and used only under the supervision of the Santa
Clarita Sheriff Station’s Watch Deputy.

The Santa Clarita
Valley Special Needs Registry is a project of the Community and Law
Enforcement Aware Response Committee (CLEAR), a Santa Clarita Valley
organization dedicated to improving safety for individuals with
disabilities. The Special Needs Registry is free to participants, and
has been funded through a grant from the Autism Society of America,
California Chapter and Los Angeles Chapter.
